What should you expect during my first visit?
On your first visit to Florida Chiropractic and Sports Rehab Centers you will receive a comprehensive neurological and orthopedic examination. This is the exact same examination you would receive at a neurologist, orthopedist or neurosurgeon. In fact, we have been told it is more in-depth than most general practitioners. The examination also includes joint and muscle function/strength tests, flexibility evaluation, and if necessary, gait analysis or other dynamic tests. X-ray will be taken if needed. Depending on each individual’s diagnosis, a rehab program, including strength and flexibility is designed specifically for each patient. We recommend you bring gym shorts and a tee shirt for the first visit. This first visit usually takes just over one hour.

to schedule an appointment. Be sure to have your health insurance information handy, as we will ask for that information when you set up your initial appointment. After setting up your appointment time, someone from our office will send you an email titled "Patient Activation PIN" which will have instructions on where you will need to go and what you will need to do to fill out your medical history forms. To decrease time during your first visit, please fill out the forms our office emails you PRIOR to your appointment. If you chose to fill out the forms at the office, it may increase your visit by anywhere from 30-90 minutes. All of our forms are electronic and the computers may not be available when your appointment time is scheduled. If this is the case, you may be requested to reschedule you appointment.
If you are coming in for treatment of injuries resulting from an accident (auto accident, slip/fall, worker's compensation, etc), you may need to sign specific forms that we have at the office which are required by Florida law.
Once logged in, complete the sections " About Me ," " New Case Intake ," and " Health History ." It will take about 20 minutes to complete and you should finish it at your own convenience before your appointment.
Failure to complete all your online forms will greatly increase the duration of your appointment.
Please make sure to provide your health insurance information (carrier and member ID number) to our office if you have not done so already, prior to your appointment so we can verify your benefits and eligibility. If we do not receive this information prior to your appointment, you will be charged the full initial office visit pricing.
Our new software will expedite your office visit, allowing our patients to have more quality time in our office. It will make our office virtual and allow you to update your personal demographics, health information, insurance, etc., as needed. With this PIN you will also be able check progress notes, reports, payments, and so on.
